Small DVR with integrated PoE - where to find/what to buy?

Those are very simplified instructions, but they'll get an electrician working. They are also by no means Best Practices either, but hey, that's why you're using an electrician and not a low-voltage tech, right? ;)

I've walked him through testing the camera plugged in directly to the DVR with a 3' patch cable I sent over. Had him test power from the separate PoE power brick and out from the patch cable - what he sent back to me:
